Insurance Policy and Payment Options at PRVNT

At PRVNT Chiropractic & Wellness, we believe in transparent care and clear expectations. We work with select insurance plans and also offer straightforward self-pay options. Because chiropractic benefits and provider networks can vary by plan, all insurance coverage is verified individually.

In-Network Insurance

We currently participate with select:

• Cigna plans with chiropractic benefits administered through American Specialty Health (ASH)

Participation and chiropractic benefits may vary depending on your specific Cigna plan, employer group, and benefit structure. We will verify your coverage whenever possible before care begins.

Florida Blue / Blue Cross Blue Shield

Florida Blue chiropractic network participation is currently being verified following recent changes to the administration of chiropractic benefits.

Because network status may vary by Florida Blue plan, we do not currently represent that PRVNT is in-network with all Florida Blue plans.

If you have Florida Blue, please provide us with your insurance information so we can verify your eligibility, benefits, and network status for your specific plan.

If PRVNT is determined to be out-of-network with your plan, you may choose to receive care at our self-pay rates. Upon request, we can provide a detailed superbill that you may submit to your insurance company for possible out-of-network reimbursement. Reimbursement is determined entirely by your individual insurance plan.

Plans That Require Individual Verification

Insurance coverage can vary significantly between plans, even when two patients have insurance through the same carrier.

Plans that may require additional verification include:

• HMO plans

• PPO and EPO plans

• Marketplace / ACA plans

• Employer-specific health plans

• Plans with chiropractic benefit carve-outs

• Plans using third-party administrators

Verification of benefits does not guarantee payment by your insurance company.

When Insurance Eligibility or Network Status Is Pending

If we are unable to confirm your insurance eligibility or network status before your appointment, you may still receive care.

The applicable visit fee may be collected at the time of service while your insurance status is being verified.

If we later confirm that PRVNT is in-network with your plan and the visit is eligible for insurance billing, we may submit the claim and apply your payment toward your final patient responsibility. Any applicable overpayment will be credited or refunded.

If PRVNT is confirmed to be out-of-network, your payment will remain subject to our self-pay/out-of-network rates, and a superbill can be provided upon request for you to submit to your insurance company for consideration of out-of-network benefits.

Self-Pay & Out-of-Network Care

You do not need to use insurance to receive care at PRVNT.

We offer:

• Transparent self-pay pricing

• Straightforward visit rates

• Care options that are not dependent on insurance authorization

• Superbill documentation for eligible out-of-network patients upon request

Many patients choose self-pay care for greater simplicity and flexibility.

Medicare & Medicare Advantage

• PRVNT Chiropractic & Wellness does not accept Original Medicare or Medicare Advantage plans.

• We are not currently scheduling visits intended to be billed through Medicare or Medicare Advantage.

• If you have Medicare or a Medicare Advantage plan, please contact the office before scheduling if you have questions about your options.

Important Insurance Information

• Insurance benefits vary by carrier, plan, employer, and network.

• Verification of eligibility or benefits is not a guarantee of payment.

• Some services offered at PRVNT may not be covered by insurance.

• Patients are responsible for deductibles, copayments, coinsurance, non-covered services, and other amounts determined to be their responsibility.

• Network participation is determined by the patient's specific insurance plan and may change.

• Patients are encouraged to confirm coverage directly with their insurance carrier when necessary.
